58 Our busy Women's & Children's Clinic is in need of a positive, motivated case worker.
The Clinic Case Worker coordinates the provision of services designed to promote patient satisfaction, cost effective quality healthcare and outcomes by enhancing the physical, psycho social and vocational health of individuals in the most appropriate setting.
Monitors patient registries to proactively arrange care and coordinate healthcare needs.
QUALIFICATIONS: Baccalaureate degree in professional nursing.
Current state licensure.
Current American Heart Association BLS certification.
Experience in case management and patient empanelment.
Certification in case management or working toward certification.
ESSENTIAL JOB REQUIREMENTS Ability to communicate therapeutically.
Able to prepare and maintain patient records and reports.
Must possess quality writing and verbal skills.
Able to react calmly and effectively in urgent and emergent situations.
Able to identify problems and recommend solutions.
Must demonstrate strong personal integrity, stamina, and emotional stability.
Ability to present medical information to insurance review organization in a concise, assertive, and effective manner, negotiating as necessary on behalf of patient, physician and clinic for approval of medically necessary resources.
