The Registered Nurse evaluates, defines, coordinates and administers the total nursing care of an assigned group of home health patients.
The RN is responsible for the written nursing care plan, acts as the patient advocate and is responsible for the direction and supervision of any personnel assigned to them.
The RN informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs.
This RN will need to be competent to care for patients of all ages.
This RN is directly responsible to the Manager of Home Care Services.
Graduate of accredited school of nursing, current Oregon RN license without restrictions minimum 1 year experience as a professional nurse in the clinical area and/or in a community health setting preferred.
Current CPR.
Prepare/maintain patient records, quality writing and verbal skills.
Establish/maintain effective working relationships with patients, caregivers, family, peers, and providers.
Able to react calmly and effectively in urgent situations.
Must demonstrate strong personal integrity, stamina, and emotional stability.
Share responsibility for 24-hour on-call on a rotational basis with other HCS nursing staff.
Have a vehicle in reliable condition with insurance coverage and a current Oregon driver's license.
Ability to communicate therapeutically and provide care for patients' age-specific, cultural, physical and psychosocial well-being.
Ability to lift 50 pounds maximum with frequent lifting/or carrying of objects weighing up to 25 pounds.
