Case Management Coordinat

This position will provide support to our case management team by assisting with inpatients authorizations, faxing clinicals and scheduling appointments.
*This is designated as a safety-sensitive position* Wage: $16.
83 - $23.
90 Responsible for assistance with discharge arrangements for care facility and home discharge.
Assist with providing clinical information to commercial insurance companies as required.
Provides support to the Case Management Team to ensure that the day to day operational functions of the department are met in a timely manner.
Assists with discharge arrangements for care facility and home discharge in collaboration with the RN &MSW Case Managers.
Responsible for timely and appropriate documentation within the electronic medical records system and Paragon Registration System.
